Filling out these forms ahead of time will help make your visits to Dr. Stefan Murgelas Family Dentistry as quick and convenient as possible. If you have any questions when filling out the form our staff will happily assist you with any questions on the day of your appointment.
Financial information
All of our patients at Dr. Stefan Murgelas Family Dentistry are entitled to a free meet and greet with Dr. Murgelas where treatment options, fees and general questions can be discussed. A dental treatment is an important decision so we want to make sure you understand exactly what is being done, the benefits, and the risks. Prior to proceeding with any treatment, fees and financial arrangements will be discussed with you and all your questions will be answered.
Dr. Stefan Murgelas Family Dentistry accepts cash, personal cheque, debit, Visa, Master Card. Payment is required at the time the services are preformed. Financing is available from a third party, inquire at the reception.
